So i need help to build a vaccination calender for children
User will give Date Of Birth of child
We will derive vaccinations dates from this date

Vaccinations are given at different ages to a child

Age 1 week —X, Y, Z vaccines

Age 4 weeks —- X A D GJ

Age 8 weeks —-++ X D G

Age 6 months —– L H I R A

and on and on

This needs to shown on user page as a table and date wise

Also we need to send reminder to the user via mail and sms
